Apr 28, 2016

File 2015-0408

On April 28, 2016, the Inquiry Committee approved a Consent Agreement following review of a medical evaluation indicating that a Non Practising Registrant who had struggled with health ailments impairing practice of the profession was fit to return to practice within specific parameters. The Consent Agreement imposes a two year limit on access and handling of narcotics, five years of medical monitoring, and disclosure to prospective employer(s).

The name of the Registrant has been withheld in accordance with section 39.3(4)(a) of the Health Professions Act for the purposes of not identifying the personal health information of the Registrant respecting the condition. The Inquiry Committee is satisfied that these terms are sufficient to protect the public.
