Consent agreement

​On September 25, 2019 a panel of the Inquiry Committee approved a Consent Agreement between BCCNP and Daniel Suboski (aka Brace) of Victoria, to remediate practice and conduct issues related to the following:

Between October 2015 and January 2017 the Registrant demonstrated multiple practice concerns while practising in a community nursing role, including:

  • Failure to visit a recently discharged client with dementia and a diagnosed infection or adequately prepare for a home visit requiring a blood pressure cuff.
  • Missing or inadequate documentation. 
  • Lack of adequate wound care and wound care planning on one occasion.
  • Inadequate clinical decision-making related to diabetes management.
  • Failure to clearly communicate regarding visits with clients, and by failing to maintain professional boundaries by drinking alcohol with a client after work hours at the client's home.

Between August 2017 and May 2018, the Registrant demonstrated multiple practice and competency concerns while practising in a coronary care unit, including:

  • Multiple medication errors, such as, multiple intravenous solutions into one line including a titrated medication, failing to adjust a patient's Heparin Infusion, failing to check the compatibility chart, failing to document administered medications, and an inability to articulate an understanding of critical care drugs, actions and effects.
  • Discussed meditation with a patient rather than assessing the patient's blood pressure.
  • Failing to know how to call a Code Blue in a critical care area, and failing to perform safety checks of emergency equipment.
  • Leaving a patient unsupervised and failed to give a report to the receiving RN at the Cardiac Catheterization Lab.
  • Performing inaccurate assessments.

On multiple occasions, between August 2017 and May 2018, the Registrant demonstrated the following conduct issues:

  • Wore earbud headphones in a critical care environment.
  • Inappropriate personal computer use.
  • Needed to be woken up from breaks.
  • Took extended breaks and returned late from breaks.
  • Failed to assess or provide documentation for his critical care patients in a timely manner.

The Registrant has voluntarily agreed to terms equivalent to a condition on practice, including: a requirement to work with supervision for a period of 12 months (including random chart and practice audits with a supervisor), and an 24-month limit precluding practice in a critical care or isolated environment or as sole RN or as the in-charge nurse, or in a supervisory capacity of any kind.  He has agreed to additional terms including disclosure to employers and remedial education.

The Inquiry Committee is satisfied that the terms will protect the public.
