​​​BCCNM requires a​ll applicants to submit two docum​ents to verify their identity.

Identification requir​​em​ents

To meet the identification requirement, the documents you submit must be valid (not expired) and include:

  • At least one primary identification (see below)
  • At least one form of identification must be government issued and include a photo
  • Your name, date of birth, signature

If it is your first time applying to BCCNM, the identification you submit must also:

  • Be notarized and verified in-person by a lawyer or notary public in their original. Virtually notarized documents are not accepted.
  • Include all current and form​er names

To protect your personal privacy, please DO NOT send the following identification documents:

  • Personal health number (PHN)
  • Social insurance number
  • Credit cards
  • Bank cards​​

Changing you​r na​me?

If your legal name has changed, please email BCCNM Registration Services at register@bccnm.ca and provide: 

  1. A copy of one piece of primary photo identification in your new na​​me; and

  2. A copy of the government issued change of name document that shows both your current name and your new name.

    OR
  1. Copies of two pieces of government issued identification in your new name, o​ne of which must be a photo identification from the primary identification.

Once the required identification is provided, your name will be updated in your BCCNM account, registrant file and in the online directory. 

Please note: Notarization is not required for name changes.

Why do I need to submit ID documents?

​We need to confirm your identity and ensure our public registry/online nurse verification includes accurate information for the public and employers. In addition to the BCCNM Bylaw requirements to verify your identity, Federal RCMP policy​ requires that we verify your identity prior to submitting your consent for a criminal record check in order to complete the required criminal record check​.

​Why do my ID documents need to be notarized (initial applicants only)?
​You must have your ID documents notarized if you:
  • Ar e registering with BCCNM for the first time, or
  • If you are applying to reinstate your registration AND your name has changed since you last held registration with BCCNM.

Visiting a notary public or lawyer with your identity documents enables BCCNM to:

  • Verify your identity
  • Prevent misidentification
  • Manage a ccess and protect the integrity and security of college records, information systems and online services.

BCCNM does not accept virtually notarized documents​.

What if my ID documents include discrepancies in my name(s)?
​​​If there is a discrepancy in your name between the two pieces of ID submitted, you must submit additional documents to explain any inconsistencies (e.g. a marriage certificate, change of name affidavit, or divorce decree). These supporting documents also must be notarized​​.

What if my ID documents were issued in another language?

For ID documents issued in languages other than English, a certified translation must be completed before you submit them to BCCNM. Please complete the following steps:

  1. Get your document(s) notarized as certified true copies
  2. Get a certified translation of your document(s).
    The translated document(s) must include the translator's official stamp and/or identification number, address and telephone number, along with the date of the translation.
  3. Submit the translated document(s) AND a copy of the document in the original language to BCCNM​.
