Consent agreement

On Aug. 28, 2025, a panel of the Inquiry Committee approved a consent agreement between BCCNM and Gregory Lazar of Port Coquitlam, B.C. to address practice issues that occurred between January and August 2024 related to practice concerns regarding following physician orders, conducting appropriate patient assessments, documenting care and assessments appropriately, following medication administration standards, and following facility protocols.

Nurses are accountable to ensure they meet BCCNM practice standards as well as meet the employer's policies for providing safe nursing care. Oftentimes when nurses are not meeting professional and practise standards, remedial and supportive measures are put in place so that a nurse will be able to address practice concerns, consolidate their knowledge and skills, while oversight or supervision measures are in place to ensure public safety.

The registrant has voluntarily agreed to terms equivalent to a limit and/or condition on their practice, including:

  1. A reprimand and a suspension of their registration for one week;
  2. Limits on their nursing registration to support the registrant while they remediate their practice, including not to be in charge, not to be the sole RN on duty, and not to supervise other nurses, for a period of six months;
  3. Requirement to practice under supervision for three months; and
  4. Remedial education targeted to address their practice concerns.

The Inquiry Committee is satisfied that the terms will address the professional and practice concerns that arose and will protect the public.
