On Feb. 2, 2026, a panel of the Inquiry Committee approved a Consent Agreement between BCCNM and Harold Jay Ayadi of Surrey, B.C., to address practice issues related to privacy and confidentiality that occurred from January 2024 to June 2025.
The conduct was unsatisfactory with respect to the following BCCNM professional and practice standards:
a. Privacy and Confidentiality
b. Responsibility and Accountability
c. Ethical Practice
Specifically, the following conduct of the registrant was unsatisfactory: accessing individual patient records without a clinical purpose on a non-random basis, failing to safeguard login credentials, and accessing his own medical records. BCCNM standards set expectations to ensure nurses are practising competently, ethically, responsibly, and safely. Nurses are accountable for their nursing decisions, actions, and professional conduct. Nurses must meet these standards in providing care to patients.
The registrant has voluntarily agreed to the following terms:
a. A suspension of their nursing registration for four weeks.
b. Public reprimand for breaching the above-noted standards.
c. Remedial education in privacy and confidentiality.
The registrant made early admissions to the privacy breaches, expressed remorse, and voluntarily enrolled in additional education.
The Inquiry Committee is satisfied that the terms will address the issues and protect the public.
