Consent agreement

Nov 3, 2015

File 2014-0522

On November 3, 2015 the Inquiry Committee approved a Consent Agreement. In 2009, a registrant self-reported an addiction to prescription medication that impaired her practice, and subsequently voluntarily entered into two separate Consent Agreements equivalent to conditions and/or limits on her practice. The Registrant cooperated with CRNBC and sought treatment for her health condition. The name of the Registrant has been withheld in accordance with section 39.3(4)(a) of the Health Professions Act for the purposes of not identifying the personal health information of the Registrant respecting the condition. The Inquiry Committee is satisfied that the terms in the Agreement are sufficient to protect the public.
