On December 16, 2014, the Registrant entered a Consent Agreement under section 36(d) of the Health Professions Act in order to ensure continuing fitness to practice. The Consent Agreement disposed of an investigation under section 33(4) of the Act regarding clinical practice and fitness to practice concerns. The Inquiry Committee is satisfied that the terms of the agreement, which require the Registrant to limit their practice to non-acute settings, complete education and counselling, and provide a written review of their practice will protect the public.
The name of the Registrant has been withheld in accordance with section 39.3 (4) (a) of the Health Professions Act for the purposes of not identifying the personal health information of the Registrant respecting the condition. The Inquiry Committee is satisfied that the undertakings will protect the public.
