On July 23, 2020 a panel of the Inquiry Committee approved a Consent Agreement between BCCNP and Jana Zajacova of Chilliwack, to remediate practice issues that occurred between June and August 2019 when the Registrant made a serious medication error when she accidentally overdosed a patient with methadone and then participated in a coordinated effort with another nurse to conceal the error. The Registrant knew that the narcotic control document was falsified and that the multidose methadone bottle was tampered with by the addition of water to it. Despite this knowledge, the Registrant failed to intervene. The Registrant failed to notify the leadership team of the medication error, failed to initiate a PSLS report and made false statements about her actions during the critical incident review.
The Registrant has voluntarily agreed to terms equivalent to a condition on practice, including: a four-month suspension followed by a four-month period of supervised "high-alert" medication dispensing upon returning to practice and disclosure of the Consent Agreement to current and future employers for oversight purposes.
The Inquiry Committee is satisfied that the terms will protect the public.
