Consent agreement

​A panel of the Inquiry Committee approved a Consent Agreement under which the Registrant agreed to undertakings to remediate practice concerns that occurred in December 2018 and February 2019 related to:

  • Failure to adhere to proper sterile technique when providing Peripherally Inserted Central Catheter (PICC) line care and failing to ensure the PICC line was clamped following a heparin lock.
  • Failure to ensure that her documentation was sufficient, including failure to adequately record and document vital signs, failure to adequately monitor and record fluid balances, failure to correctly calculate and record a Pediatric Early Warning System (PEWS) score and a Pediatric Respiratory Assessment Measure (PRAM), failure to document a suspicious rash and failure to document a cardiovascular assessment on a patient with a sustained increased heart rate, including failure to place a patient on continuous electrocardiogram (ECG) monitoring.
  • Failure to correctly discontinue a medication order.

The Registrant underwent an Independent Medical Evaluation (IME) which identified a nexus between the clinical concerns and medical conditions.  The Registrant has voluntarily agreed to terms equivalent to limits and conditions on practice, including: a condition requiring a period of oversight/supervision in the workplace, a limit precluding working night shifts for 12 months, and remedial activities including education and developing a Learning Plan. The Registrant agreed to a reprimand for the concerns noted with her nursing practice. The undertakings also include regular monitoring of the Registrant's fitness to practise nursing regular reports from treating physicians regarding compliance with treatment recommendations and disclosure of treatment recommendations to relevant employer representative(s).  The Agreement will remain in place for a minimum of four years of continuous full-time nursing practice.

The name of the Registrant has been withheld in accordance with section 39.3 (4) (a) of the Health Professions Act for the purposes of not identifying the personal health information of the Registrant respecting the condition. The Inquiry Committee is satisfied that the undertakings will protect the public.​​
