To protect your personal privacy, any documents containing the information below will be redacted from our system. Please DO NOT send the following identification documents or you may be asked to submit an additional notarized document:

  • Personal health number (PHN)
  • ​Social insurance number
  • Credit cards
  • Bank cards​

Identification docume​nts must:​

  • ​​​Be valid and unexpired
  • Be notarized and verified in-person by a lawyer or notary public in their original form; BCCNM does not accept virtually notarized documents

  • Include at least one primary identification

  • Include all current and former names

  • Include at least one government issued and photo ID

  • Include your name, date of birth, signature

Frequently asked questions

Why do I need to submit ID documents?
​We need to confirm your identity and ensure our public registry/online nurse verification includes accurate information for the public and employers. In addition to the BCCNM Bylaw requirements to verify your identity, Federal RCMP policy​ requires that we verify your identity prior to submitting your consent for a criminal record check in order to complete the required criminal record check.
Why do my ID documents need to be notarized?

Visiting a notary public or lawyer with your identity documents enables BCCNM to:

  • Verify your identity
  • Prevent misidentification
  • Manage access and protect the integrity and security of college records, information systems and online services.

Submission of notarized documents is also one of the requirements for registration outlined in BCCNM’s bylaws.​

BCCNM does not accept virtually notarized documents​.

What if my ID documents include discrepancies in my name(s)?
​If there is a discrepancy in your name between the two pieces of ID submitted, you must submit additional documents to explain any inconsistencies (e.g. a marriage certificate, change of name affidavit, or divorce decree). These supporting documents also must be notarized.​
What if my ID documents were issued in another language?
​For ID documents issued in languages other than English, a certified translation must be completed before you submit them to BCCNM. Please complete the following steps:
  1. Get your document(s) notarized as certified true copies
  2. Get a certified translation of your document(s).
    The translated document(s) must include the translator’s official stamp and/or identification number, address and telephone number, along with the date of the translation.
  3. Submit the translated document(s) AND a copy of the document in the original language to BCCNM.
